For Physicians

Overview

Home health care enables your patients to recuperate from surgery, short-term illness or deal with chronic disease in the comfort of their own homes. At UR Medicine Home Care we seek the best patient outcomes for everyone in our care, and consistently meet or exceed regional, state and national benchmarks. We work closely with you to provide your patients and their families the care, education, and support needed for them to become independent in their care. Our skilled team of home health professionals will monitor your patient’s physical, social, and emotional needs and make sure to communicate quickly with you should their status worsen or decline.

Certain insurances require a patient to have a home bound status. Home bound is defined as individuals who cannot leave their home regularly without difficulty and without assistance. Services and programs are based on the needs of your patients.
